Medical Release /Indemnity Form
Everyone who participates in any children's activity during the year, including VBS, camps, retreats, etc. is required to have an medical release and indemnity agreement. One form per child is good for an entire year running from May to May. If you would like to print out a copy of this year's agreement for children, click here (pdf format). The form does need to be signed in the presence of a notary. The church has notaries on staff who will witness your signature at no charge.
