Direct debit contribution form June 6, 2008 - For your convenience, Wilshire offers a direct debit of your contributions from your checking account either monthly or twice-a-month. To take advantage of this option, print out the Pre-Authorized Payment Agreement, fill it out, and mail it with a voided check to: Teresa Meyer Financial Assistant Wilshire Baptist Church 4316 Abrams Road Dallas, TX 75214 Or deliver it to the church office addressed to Teresa Meyer or Paul Johnson, Minister of Business Administration.
